Problem with very simple template

Hi I  have problem with very simple template - there is only 2 fields with the same name in site code and accept button. Can somebody PM and help?

    Ok, so I have problem with template on this site http://www.pracadokitu.com/forum.php?a=pc&id=1795 I'm checking several options and all the time there is a message "no form at all" :<

    I didnt test it in SER but most likely there is no form but javascript you should use post data=
