How to add image and also hyperlink in message box

How can I add image in message box 

Also how would I be able to add hyperlink  in message box like make click here clickable (hyperlink)

You can't. Email forms are sent as plain text. The best you can do is a plain url that the user's email client should turn into a clickable link.
