Theres no set definition and its means different things to different people so can be a nightmare when people describe one of the tiers as a Kitchen Sink.
What i have understood is "Selecting Every Engine on GSA and wasting resources and Time" is called Kitchen Sink.

So if you are packing up a suitcase and its really heavy and someone says, what did you pack in here, everything but the kitchen sink? It a funny saying that just assumes they packed way too much more then they needed. In other words everything imaginable.

Kitchen sink means everything.But like @shaun said 'all' depends with each user.I don't do KS. I filled up gsa ser data manually myself and build 50 - 100 backlinks per day. 2 tiers works fine for me.
