POP Up verification style box

I am running my projects with my GSA Captcha breaker running.

Which is all running fine, but i keep getting a pop up box that looks like a verification of some type asking me things like what country is new York in, male or female question, 9+2 and many, many more random questions.

What is this and how do i deal with it?

@seomarketer2011 go to the options tab in your project and select random for "If a form cannot be filled" option
