SER bad html code

I was going through some of the articles SER has made. some of them are ok, but in some of them, the html is wrong. for example, it makes the backlink to http://www.example.com"> instead of http://www.example.com
and the <a href is still there....and there is also a lot of <br> in there. This problem is happening with about half of my verifieds. why is it doing this? in the options tab, I have my urls set like this:
http://www.example.com#click here#more information#visit site
Isn't that right?

I need the URL where you see this. Might not be bad html code but the site not accepting the html at all.
